Providing essential support for member transitions in the community, the full-time Behavioral Health Transition Specialist will coordinate care plans, educate enrollees about services, and assist with member assessments while working remotely from Texas. Key responsibilities Supports care coordination teams and providers in developing effective transition plans for members Assists with the transition process for members based on enrollment or changes in care services Educates members and their families on service requirements, limitations, and exclusions during the transition planning process Required qualifications Bachelor's degree in a relevant field 2-4 years of related experience in behavioral health or care coordination Experience in patient or member-facing roles Strong understanding of care transition processes Ability to engage effectively with patients, members, and healthcare providers
